Patient Rights and Responsibilities
Purpose: This document is meant to clearly identify the rights of all patients recognizing the dignity and vulnerability of each person as a patient who receives care at Hind General Hospital It also lists the patient’s responsibilities in order for us to contribute to more effective care and greater satisfaction
Patient Rights:
- You have the right to considerate, safe and respectful care, including the right to have your cultural, spiritual, emotional and psychosocial needs addressed, and to be respected for your personal values, beliefs and preferences.
- You have the right to receive pastoral and spiritual care and or services.
- You have the right to be informed about your diagnosis, prognosis, treatment and services, including plan of care and unanticipated outcomes, in language and words you can understand
- You have the right to effective communication services, including translation and interpretation, and you have the right to consideration of any special needs that relate to vision, hearing, speech, language and cognition impairment.
- You have the right to know the names and titles of all persons who provide care to you from admission to discharge. Names and titles of bedside caregivers will be kept current in your room with each shift change.
- You have the right to know the immediate and long-term financial implications of treatment choices, insofar as they are known.
- You have the right to be involved in decisions about your plan of care, treatment and services provided. To the extent provided by law and regulation, you have the right to refuse any recommended treatment. You will be informed of the effect any specific refusal will have upon your health.
- You have the right to have your family involved in the decision making process whenever you are unable to make your own decision about care.
- You have the right to be informed about advance directives and the process for appointing someone to make healthcare decisions (a surrogate) for you if you become unable to speak for yourself. The hospital will honor your valid advance directive to the fullest extent possible whenever it is presented. If you have none but wish to complete an advance directive, a hospital representative will assist you.
- You have the right to every consideration to preserve your privacy, safety, security, and personal dignity. Discussion of care, consultation, examination and treatment will respect that right. You also have the right to telephone privacy.
- You have the right to confidentiality in the handling of your health information except when reporting is required by law, as in suspicion of abuse or public health hazards.
- You have the right to review records pertaining to your care, and to have that information explained and interpreted as necessary, except where restricted by law. You have the right to request an amendment to, and receive disclosure of, your health information.
- You have the right to have recording or filming of your care and treatment done only with your written consent, and then, only in accordance with the law and hospital policy.
- You have the right to continuity of care, and the right to be informed of realistic and available options when hospital care is no longer available.
- You have the right to refuse care, treatment (informed consent), and services in accordance with law and regulation.
- You have the right to issue a complaint freely, and to have your complaint promptly investigated and satisfactorily resolved. You are entitles to information regarding your right to file a complaint with the State authority without risk of coercion, discrimination, reprisal, or unreasonable interruption of care, treatment and services.
- You have the right to receive support from the hospital Patient Advocate whenever you have hospital-related problems or concerns.
- You have the right to have your pain managed according to the highest standards of competent medical practice.
- You have the right to a safe and secure environment for yourself, your property, and your visitors.
- You have the right to protection from neglect and abuse by hospital staff, students, volunteers, other patients, visitors and family members. If you ever feel protection is needed you have the right to receive information regarding advocacy and protective services.
- You have the right to freedom from restraints, except in the case of medical necessity, and only in accordance with the law.
- You have the right to make your wishes known regarding end-of-life treatment and organ donation, and to have those wishes honored within the limits of the law.
- You have the right to safe and competent care , treatment and services, regardless of your ability to pay, insofar as it is compatible with the law, and within the scope of the hospital’s Mission, and its ability to provide
Patient Responsibilities:
- You have the responsibility to provide complete and truthful Information regarding your health, including present complaints, past Hospitalizations, and medication you have you used or are currently using.
- You have the responsibility to follow the instructions of your caregivers, and to accept any consequences which result from your failure to do so.
- You have the responsibility to report any concerns or changes in your condition (includes pain management).
- You have the responsibility to ask questions whenever you do not understand what you have been told, or what is happening.
- If you have an advance directive, you have the responsibility to provide a copy to the hospital.
- You have the responsibility to inform your physician an any other appropriate healthcare providers if you anticipate difficulty in following your prescribed treatment.
- You have the responsibility to follow the organization’s rules and regulations.
- You have the responsibility to meet your financial obligation to the hospital. That responsibility includes the provision of information necessary for filling insurance claims, and cooperation with the hospital when other payment arrangements are necessary.
- You have the responsibility to be respectful and considerate of the rights and property of other patients, and of all caregivers, staff, and volunteers.
- You are responsible to report anything you believe is unsafe in regard to how your caregivers are providing your care and with your physical environment.
Important Information for Patients and Families
If any patient, or family member has a concern about the care, or wishes to discuss an ethical issue related to treatment, or if they wish to file a grievance concerning their care, they are encouraged to inform one of their caregivers or call the Patient Advocate at 219-947-3030 ext. 263.
